Health Information Sharing Authorization
Beacon Boxx, Inc. · Effective 11/1/2026
I, the Beacon Boxx user who clicks the acknowledgment box in the app (“I,” “me”), authorize Beacon Boxx, Inc. (“Beacon Boxx”) to use and disclose the information about me described below (“My Health Information”) as set out in this Authorization.
1. Information Covered
Vital signs and biometric readings from my connected wearable or devices (for example heart rate, blood oxygen, activity, and sleep data).
Emergency alert and event records, including SOS presses, AI-detected events, inactivity (dead-man) triggers, dismissals, and the response timeline.
My location at the time of an alert, and my home address.
Health and safety profile details I choose to enter, such as medical notes, medications, allergies, physician contacts, and access instructions for my home.
My name, age, phone number, and account details needed to identify me to recipients.
Not included. This Authorization does not cover psychotherapy notes, substance use disorder treatment records, HIV or AIDS test results, or genetic test results. Beacon Boxx will not share those unless I accept a separate authorization specific to them.
2. Who May Receive It
Part A. Required for emergency monitoring. I authorize Beacon Boxx to disclose My Health Information to:
Beacon Boxx’s contracted emergency monitoring center, located in the United States or Canada;
911 and emergency medical services, police, and fire responders dispatched to help me; and
the emergency contacts I list in the app, by in-app message and SMS, when an alert occurs.
Part B. Recipients I choose. I authorize Beacon Boxx to share My Health Information with each person or organization I add in the app, at the access level I select for each one. Recipients may include:
Recipient type
Example
Access levels I can choose
Caretaker
Adult child, spouse, friend
Alerts only, or alerts plus live vitals and history
Healthcare provider
My physician or care team
Summary reports, or full history
Other person or organization
Home health agency, attorney, care manager
Alerts only, summary, or full history
The recipient’s name, contact details, access level, and date added are recorded in the app and in my sharing log. I can change any recipient’s access at any time.
3. Purpose, Term, and Revocation
3.1 Purpose. Part A disclosures are made to detect emergencies and get me help. Part B disclosures are made at my request, so that people I choose can support my health and safety.
3.2 Expiration. This Authorization stays in effect until the earlier of (a) the date I revoke it or (b) the date my Beacon Boxx account is closed. Access for an individual Part B recipient ends when I remove that recipient.
3.3 My right to revoke. I may revoke this Authorization, in whole or for any one recipient, at any time by (a) removing the recipient or turning off sharing in the app, or (b) sending a written request to support@thebeaconboxx.com. Revocation takes effect when Beacon Boxx receives it. It does not undo any disclosure already made in reliance on this Authorization.
3.4 Effect of revoking Part A. If I revoke Part A, Beacon Boxx cannot send alerts to the monitoring center, responders, or my emergency contacts, and emergency monitoring will stop. Beacon Boxx will warn me in the app before this takes effect.
4. Required Statements
4.1 Redisclosure. I understand that once My Health Information is shared with a recipient I choose, it may no longer be protected by HIPAA or other privacy laws, and the recipient may share it further. Beacon Boxx is not responsible for what a recipient does with information I directed Beacon Boxx to share.
4.2 No conditioning on Part B. Beacon Boxx will not condition my subscription, monitoring service, or eligibility on my accepting Part B. Part B is entirely optional.
4.3 Part A is part of the service. I understand that Part A is necessary to provide emergency monitoring. If I do not accept Part A, Beacon Boxx cannot provide monitoring, but I may still use non-emergency features where available.
4.4 No sale. Beacon Boxx does not sell My Health Information and will not use this Authorization to share it for advertising or marketing. Beacon Boxx receives no payment from any recipient for disclosures under this Authorization.
4.5 Access to this document. This Authorization is published at www.beaconboxx.com/legal/hipaa-authorization and linked from the app at all times. I may view, save, or print it at any time, and the version I acknowledged is available on request.
4.6 Electronic acceptance. Clicking the acknowledgment box in the Beacon Boxx app is my agreement to this Authorization. I agree it is legally binding and has the same effect as agreeing in writing.
5. Emergency Sharing Terms
5.1 What happens in an alert. When an alert is not dismissed within the in-app dismissal window, Beacon Boxx sends my alert, location, vitals at the time of the event, and relevant health profile details to the monitoring center. The monitoring center may pass that information to 911 and responders. My emergency contacts are notified at the same time.
5.2 Monitoring partner obligations. Beacon Boxx’s monitoring partner is bound by a written agreement to use My Health Information only to respond to my alerts, to keep it confidential and secure, and to keep all operators and data in the United States or Canada.
5.3 Life-threatening situations. If Beacon Boxx or its monitoring partner believes in good faith that I face a serious and imminent threat to my health or safety, they may share information reasonably needed to get me help, as permitted by law, even with someone not listed in this Authorization.
5.4 Records. Beacon Boxx keeps a log of each disclosure (date, recipient, and type of information) and will give me a copy on request at support@thebeaconboxx.com.
